I did read a fair bit of similar problems on this forum, and I'm not looking for an immediate solution, but rather help with diagnosing the problem:

As we are on the road (in Italy, our van's home country) currently I don't have all options available to me to work on it... So this is what happened with our X250 3.0 2007 today.
I was cruising on a fairly flat road doing about 70 km/h in 5th (so not too much load) when the engine suddenly cut out with the MIL coming on. I have a little BT ODB dongle with me that told me error code P0089. The engine restarted and I continued driving, I also cleared the code to see if it came back. I then wanted to overtake a tractor a few km later doing 30km/h and just when tapping the accelerator (or shifting back) the engine cut out again. This time I just restarted it while driving and overtook the tractor.
We then drove another 70km including very steep climbs of 10% and more without any problem. The van drives like it always does. We have quite some cargo (it's a camper van) so we needed every bit of power from the engine to get up the mountain.

We now parked up for the night and I am wondering what I can do here to find the issue. I have tools with me (but no power tools) and I could disassemble the front to get to the fuel pump and regulator.
The fuel filter was replaced before we left (about 3000 km ago).
With Car Scanner and the OBD dongle I saw fuel pressure between 300 bar and 1400 bar, revving the engine while idling, I read in the training manual the fuel pump is directly driven by the engine so I guess these readings are normal. While idling pressure is stable.
Since the engine can be restarted immediately I would suspect an electrical issue, or faulty sensor. Even the solenoid regulator could be an issue.
Is there anything I can check/diagnose/clean while on the road? Or should I just leave it alone and drive to a dealer if it keeps coming up?
 
Remembered reading this, wrong fuel filter fitted.

 
It does seem similar indeed. Although why would it happen now? We drove all the way from Belgium through the French alps into Italy... We just climbed another steep hill. I don't want to rule it out but it seems strange. I would think this would then happen under load.
Btw, after reading the article linked in the last post: the throttle body is already replaced...

I have a Mahle KX 208D filter atm
 
Did another drive today without any issues. About 100km with steep descends and ascends and some light cruising in between.
Fuel pressure fluctuates between 350 bar and 1550 bar. Mostly it stays into the 500 - 1300 bar range. Each time I release the accelerator the pressure drops down to these lower values. I assume this is to be expected and this is when the solenoid fuel pressure regulator kicks in?
 
Sorry but instead of posting a new thread - any updates on this?

Got a 2017 2.3 Ducato throwing a few different errors (mainly p0089 but also a few more intermittent random one-off's). Going in to limp mode every time I try to increase power in higher speeds. P0089 with various descriptions. Checked the fuel filer, dark color and some minor 1mm-sized dirt coming out when dipping it in diesel. Ordered new one and a new fuel pump assembly. (filter was replaced 5000km's ago. but was a cheaper one.. 15 Euro or so). hoping its just the filter.

But @nigelvan : what helped your situation in the end?
 



Possibly your second last line of defence filter for high pressure pump clogged.

Well the problem never came back. I blame it on crappy fuel...

Had a quick look at my replacement engine. Does not seem like that filter is accessable on my pump.?

Check the number on your pump matches the numbers internal filter is listed to fit.

It has been reported sometimes a similar filter has been found inside the fuel pipe delivering fuel to the high pressure pump. Or returning fuel to tank.These in pipe filters can also clog.

Unscrew the fuel inlet banjo bolt, look in the bolt and inside pump where bolt screws

I have looked into the internal filter for this type of pump further.....
This filter may be filtering the fuel before it returns to the tank.
In the video a pressure gauge is used, I believe it may be teed into the line between the main filter and the hp pump.
Reading Over 6 bar pressure with blocked internal filter. About 4.5 bar after internal filter replaced.
It appears blocked internal filter may cause rail pressure to be higher than target pressure , enough to trigger p0089.

With a scan tool showing live data watching the desired fuel pressure and the actual pressure under the conditions that the fault occurs could indicate cause of problem.
 
Thanks for the replies. So Seems like this is pressed in on my extra pump. Attaching better pictures. the pump number is in the spare part catalog that was posted previously so there seem to be afilter in there and the reasoning fits - pressure back is hindered increasing the pressure to the high pressure pump. Still waiting for a new fuel pump + new filter to rule these out but if not solved with those changes I will try pulling up this "rivet" with something whatever I can screw in and make it pullable. Will change the timing belt soon so its a perfect job to do at that time.

In a video someone removes the plug by tapping a 7mm hex key in and using an unscrewing action.
Tapping a torx key in may work too.
 
Allright, so I got the plug opened and inspected the filter on the extra pump and the pump in the car. Both filters looked flawless. Havent recieved my new regular fuel filter yet so only theory left is that that filter was clogged and somehow made the error codes. Adding some pictures for anyone about to open that filter up, (I believe I found a great tool for the job.)

Another question from this error code situation - while this code occurs the engine takes quite some time to start - "several revolutions".. or perhaps what feels like 4 seconds of starter motor. Otherwise it starts instantly. Would that change the theory of fuel filter clogged?
 
In your first post you mention other "random" fault codes ........
Check all the vehicle ground wires
