Hi all I have a 2005 fiat ducato maxi with a 2.3L engine, come to start it and its sounds like a miss fire but if you rev the engine past 1200 rpm it's totally fine, but as soon as it returns to idle it's misfiring again, any idea?

Piston/bore wear may do that? As revs increase pistons pump better/ lose less pressure.
Any white exhaust smoke like unburnt fuel especially from cold, or blue smoke oil burning?
Generally if too much fuel, you get black smoke but engine quite likes that, though the car behind may not.;)

Thank you for your reply sadly no signs of smoke at all, I wonder if I have a hole in the exhaust as I can here the turbo spinning

Just another little joyous thought, have you got a diagnostic tool that can show fuel pressure at the common rail.
I recently fitted a S/H injector pump on a 2010 2litre Fiat Scudo (Peugeot Engine) which cured a misfire and poor running due to low pressure.
Still if Turbo sound noticeable it may be worth checking for a leak in intercooler and pipes etc. Although I can't see that giving a physical misfire.
