Sound Error

Recently my computer has started acting up. I haven't changed ANYTHING but now when i'm watching YouTube videos, playing games or listening to music the sound occasionally glitches, skips and makes horrible distortion noises, which sometimes also freezes the program playing the media. When gaming it causes visual lag while the noises are happening.

When it crashes in Windows Media Player it says;
"An audio device was disconnected or reconfigured. Verify that the audio device is connected, and then try to play the item again."

But i'm pretty sure it's as solidly connected as it can possibly be.

What is going on!?

Tried reinstalling the audio driver?
Checked the graphics card (if separate) is seated correctly and the cooling fan (if any) is running correctly?

My laptop is doing the same thing! started this week.
No error messages logged..
