General loss of power

I have a ducato 2.8 d 2001, now I cant get above 10 times the gear. so that would be 10 miles in first, 20 in second and so on. it puffs out smoke and gives up a bit when it dose this.
any ideas!

not enough information there but im thinking injectors or fueling of some sort,
